Eligibility: Academy students and AD Cadre
Appointments: Please call the front desk at 915-742-8013 or visit the clinic for assistance. Patients are seen by appointment only.
Assessment Services:
- Comprehensive blood work interpretation and education
- Medication reconciliation and refills
- Hypertension management (new prescription and/or refills)
- Elevated cholesterol management (new prescription and/or refills)
- Limited referrals based on assessment findings
Non-assessment related services:
- PHA
- MEDPROS vision update
- HIV lab order
- DD Form 5118 screening
Students are assigned a Primary Care Provider (PCP) at Mendoza Clinic, please establish care accordingly for any chronic care management.
