Mission

The SLS mission is to provide Senior Leaders with a highly personal and customized medical assessment and educational environment to enhance their understanding of the complex interplay between leadership, health, fitness, and readiness.

Goals

Optimal leadership performance and disease risk mitigation will be achieved by integrating an approach where senior leaders receive an extensive clinical diagnostic evaluation, aggregate risk factor analysis, and prevention strategies, led by experienced, credentialed professionals.

Assesment Preparation

Day before: abstain from strenuous physical activity.

Night before: abstain from alcohol.

Morning of appt: abstain from food, fluid, and exercise prior to arriving to SLS for your assessment. Please feel free to bring coffee, water, etc. and snacks to consume after a staff member clears you to do so.

Please arrive to appt in your respective service PT uniform to include spandex underneath. Feel free to bring in your duty uniform to change into after you have completed the assessment.

Assesment Day

Please check in 10 minutes prior to appointment time

Station 1: Vitals will be taken (blood pressure, pulse, etc)

Station 2: Seen by an Occupational Therapist

Station 3: Seen by a Registered Dietitian

Station 4: Seen by Physical Therapist, Exercise Physiologist or PT Tech

Station 5: Meet with Physician Assistant or Nurse Practitioner

Station 6: Meet with Registered Nurse for assessment results

Based on your results, you may be referred for follow up care

Eligibility: Academy students and AD Cadre​

Appointments: Please call the front desk at 915-742-8013 or visit the clinic for assistance. Patients are seen by appointment only.​

Assessment Services:​

  • Comprehensive blood work interpretation and education
  • Medication reconciliation and refills
  • Hypertension management (new prescription and/or refills)
  • Elevated cholesterol management (new prescription and/or refills)
  • Limited referrals based on assessment findings

Non-assessment related services:​

  • PHA
  • MEDPROS vision update
  • HIV lab order
  • DD Form 5118 screening

Students are assigned a Primary Care Provider (PCP) at Mendoza Clinic, please establish care accordingly for any chronic care management.

  • Biofeedback protocol and device training (HeartMath: Quick Coherence Technique, Heart Focused Breathing)
  • Personal Resilience training
  • Sleep evaluation
  • Sleep optimization training

​Services Provided:

  • Outpatient physical therapy for all musculoskeletal conditions

Services Provided:​

  • Nutrition interventions and recommendations are unique to the individual based on their lifestyle and health goals.
  • The Registered Dietitian will work with you to make diet and lifestyle changes for favorable health outcomes.
  • Registered Dietitian will not give you a meal plan. You will get comprehensive sessions to facilitate long term, sustainable change based on your needs.

If you are due a visual screening in MEDPROS you may come to the SLS between 0800-1400 hours or contact Mendoza to schedule an appointment at Clinic 915-742-1460 or 915-742-3292.

If you wear glasses, bring a pair with you for the screening.

Remove contact lenses at least two hours prior to testing.

Those ages 45 or older will be required to complete a near vision test.
